Applying for re-evaluation due to worsening of medical condition


A person for whom a degree of disability has been determined and whose medical condition related to the work injury recognized by us worsened, or if an new impairment caused by the recognized injury is discovered - can submit a worsening condition claim to obtain a higher degree of disability.

Conditions for submitting a worsening condition claim

In order to qualify for the evaluation of your medical condition caused by the recognized injury, you need to meet the following 2 conditions:

  • You were determined a permanent a permanent or temporary degree of disability.
  • 6 months passed since the date of determination of disability degree (in special cases, the National Insurance's physician is allowed to approve reexamination appraisal before the end of this period).

Filing a worsening condition claim

1.  A worsening condition claim form (in Hebrew) must be filed.

2.  Required attachments to the claim include either a worsening condition certificate signed by a duly authorized health maintenance organization (HMO) physician or hospitalization discharge summary (if you were hospitalized due to the worsening, you may submit a discharge summary up to 90 days since discharge date, without the need for worsening condition certificate by an authorized HMO physician).

Important information!
Without the above two documents, we will not be able to process your claim.

What is a worsening condition certificate delivered by a HMO physician, and how can it be obtained?

You need to contact your HMO secretary's office and request a worsening condition certificate as victim of a work injury from a physician authorized to deliver such document on behalf of the HMO.

Please note that taking a direct appointment with a physician is not necessary, this request is handled by the secretary's office.

Upon your inquiry at the HMO, you have to bring the following documents:

  • Last Medical Board protocol determining your degree of disability (a copy of the protocol can by printed from your Personal Service account or delivered at one of our branches).
  • An up-to-date medical certificate from a specialist physician in the field related to your claimed worsening. For instance: for a back injury (orthopedic injury) you need to produce a medical certificate from professional orthopedic physician.

Request's handling process at the HMO

The HMO's secretary will forward the attached medical documentation for review by the physician authorized for this purpose by the HMO.

The authorized physician will review the documents at hand and decide:

1. whether or not a worsening of the work injury has occurred.

2. whether you need to be invited to a medical examination (an appointment will be scheduled by the HMO secretary's office).
